What's Inside the Equine SOAP Template

Our Equine SOAP Template was designed with the realities of large animal and equine practice in mind. It goes well beyond a generic SOAP structure to capture the details that matter in equine medicine. Here's a section-by-section look at what's included.

Subjective

The Subjective section covers the full patient history: animal ID, signalment, use or role, and farm or herd details if applicable. From there it captures the presenting complaint, a timeline of the current issue, and any treatments already attempted. It also prompts for herd history, management and environment, diet and nutrition, current medications, travel history, changes in behaviour or production, elimination, and reproductive history. Sections that don't apply are automatically excluded from the final note, so your note stays relevant to the case.

Objective

The Objective section is structured around a complete equine physical exam. Vitals (TPR, weight, BCS), then a head-to-tail systems review: general appearance, hydration, mucous membranes, cardiovascular, respiratory, GI, musculoskeletal and lameness, integument, neurologic, and reproductive or mammary. There's also a diagnostics subsection for bloodwork, fecal analysis, cultures, imaging, and any other procedures performed.

Assessment

The Assessment section is structured to capture your clinical reasoning clearly. It includes a problem list, primary diagnosis, differential diagnoses, and a prognosis field with standard categories (good, fair, guarded, or poor). Like other sections, fields will be automatically removed if they don't apply to a particular case.

Plan

The Plan section covers treatment details including medications, dosages, routes, and duration, with a prompt to clearly distinguish between new prescriptions and refills and to flag any changes. Sections for management and husbandry recommendations, nutrition, and follow-up instructions round it out. There's also a built-in client communication section that generates a professional follow-up email summarizing the visit if needed, so your clients stay informed without you drafting another message from scratch.
